I think they are past weapons that drop from new NMs.

if im correct, they turn into the blue sea type weapons, with help from the synergy. The hard part i heard is getting the hnm dead, to get the damaged weapon of what your going for. With a good set of ppl, you can get those formers down.

No, the Lyft weapons are from a different set of mobs, and have a hidden effect apparently based on how many people are in your party. There's a thread on BG regarding them.

The Fomor NMs, on the other hand, drop a variety of chipped, dull, etc weapons, that can be synthed through Synergy into the weird white/blue weapons.
