Even with my paragon practically maxed out, I could never get Miranda to "Be the bigger woman". I never got her loyalty back, and she was the only one that died at the end. I had Legion in the vents, Garrus leading the fireteams, Jacob escorting the crew back, and Jack doing the shield.

Practically maxed out won't cut it for that check. It has to be maxed out. If you are using a class that only gets a 70% paragon/renegade boost, spend the 3000 element zero to switch to the class with the 100% boost, and that just might do the trick. I used exactly that so I could pass the initial Miranda/Jack confrontation (and for Zaeed's paragon path), as Jack and Zaeed's missions are effortless, so the loss of my preferred class wasn't much of a detriment. Then I switched back afterwards and carried on with the game.
