Good luck with your fights, and no you don't have to talk to him before every fight. Just take your test and trade it to the correct burning circle for the fight. I think wiki has a list of which jobs are at which burning circles.

Yes, once you complete the battle for the 15 jobs you should just return to Maat. If it isn't working, is it possible you did miss one fight? Hope you can figure it out, here's a list of the jobs/BC I found. I assume you've already done most of them, hopefully you can get that last one!

Simba, make sure you're talking to him at the end on a job that is one of the fifteen you can fight him with.
Obvious, but some people talk to him on a ToAU/WotG job and kinda forget that to get the cap, it needs to be on a job you fought against him with.
Also, every fight, you just have to win. Did you have like a checklist for wins so you're sure you beat him with each and every job?
I think it's like Kairi said and a job was missed or forgotten.
If you're confident you've won with every job, have you ever been hacked and had a rollback to a certain date? If so maybe a victory obtained was wiped.
Don't know if you care, but I've always wanted to share my track record with someone else who seems to have done this legit (or mostly legit since idk if you've SMN burned any of the latest).
My pattern was like this:
1. RDM - 1 May '07 – 1/5
2. PLD - 1 June '07 – 1/2
3. NIN - 15 April ’08 – 1/1
4. DRK - 16 July ’08 – 1/2
5. WHM - 18 July ’08 – 1/1
6. MNK - 12 Oct ’08 – 1/2 (Win @ 75)
7. WAR -10 Feb ’09 – 1/1
8. DRG -30 Mar ’09 – 1/1
9. SAM - 1 Apr ’09 – 1/2
10. BLM - 14 Apr ’09 – 1/3
11. BST – 29 May ’09 – 1/2
12. THF – 29 May ’09 – 1/2
13. BRD – ?? May/June ’09 1/1
14. SMN – 23 June ’09 1/1
15. RNG – 26 June ’09 1/1
If worse comes to worse, just cross off a list of the ones you're 100% you've done and challenge him again on the ones you're unsure about and talk with him.
