I just saw a great tearjerker of an episode on Scrubs.
I don't normally watch TV in my spare time nor do I tear up during medical drama shows, but this one got me.
A brief summary: It begins in a supermarket where Dr. Cox and Dr. Dorian meet up with an old patient that was committed to the hospital because she tried to commit suicide. The thing is, she is so bipolar and chipper most of the time in the publics eye and become quite annoying to be around. So Dr. Cox bails on both Dr. Dorian and Jill leaving Dr. Dorian with the awkward conversation to Jill.
Let me skip a bit further in the episode near the end where Jill was recommitted to the hospital due to her recent attempt of committing suicide by overdose. After a few hours of trying to keep her alive, she eventually dies. Dr. Dorian takes this upon himself and becomes depressed. Dr. Cox offered a bit of advice to Dr. Dorian in hopes of curing his depression. He says, "The second you start blaming yourself for peoples' deaths there's no coming back".... let me stop here. I did some searching and youtube has this part I'm describing on it. (who knew)
