My dad have one of those from his left shoulder down to the thumb/index finger.

Its called scarification. They make deep cuts, and once they've made it in the desired pattern, you pour some salt mix on it and wrap it in plastic so that it have a hard time healing, thus creating a highly visible scar. Rather cool, actually xD

They offcourse have to do it without pain killers to show themselves macho.
