Could be. I'd try putting in a CD that you don't care about and seeing if it happens to that CD to make sure that it's the drive and not just the CD.

if you tilt your 360 while spinning a disk youll rip a hole right through it, that fucking lense is sharp.

Its also more prone to happen if you have it standing upright instead of laying flat. Friend had a disc break while the game was running. It literaly shattered while spinning at max speed. Shards tore up the whole inside of the thing.
