My Day

Today was a class day. I have a test in OB and Med/Surg tomorrow LOL. Being gimp sucks, but the rumor going around school was that I fell into a manhole, which made my day.

Today in OB we did role play activities and I kept getting asked to be the patient in labor. I think I overdid the last skit a bit because the teacher stopped asking me to be the patient after that. I kept asking my nurses for massages and frequently demanded pain medication and threw things at my pretend husband.

In med/surg we started a chapter on advanced Cardiovascular stuff. Did you know that the heart is two electrical pumps? SA node is like the pacemaker, it fires off and the AV gets the signal and it shoots to some other node that I'm too tired to remeber, then it goes to the Bundle of His, down through the Perkinji Fibers which makes the ventricals contract.

LOL I bet you think that is "advanced" but it is basic anatomy and physiology which I found out today that I need to brush up on a bit. I'd do it tonight but the two tests are getting in my way. Unfortunately, our MS instructor doesn't test after we finish getting lectured. So I'm being tested on material we learned last week and being lectured on Cardiology which sort of makes my brain hurt.

Yay for Friday!!!!!

The sinoatrial (sp?) node IS the pacemaker, Kink. I learned all that stuff last year in anatomy back in high school. You'd be surprised how much you can learn in an anatomy class. In fact, thats the only reason I knew what the hell the docs were talking about when they told me about my hernia, lol.

Update!

Today I landed in the ER which was really interesting. My day started of when another student and I were given a list of patients to transfer to the "holding area" from the E.R. because the hospital has no beds.

Unhook pulse oxemeters, ekg leads, automatic B/P cuffs and other assorted wires, move the patient to a room 20 feet away and hook every thing back up lol. This took about two hours.

I had a chance to insert a foley catheter today but the doctor's order wasn't in when my instructor came by to watch me do it :banghead3:

Now my day really starts when the charge nurse of the ER asks my instructor who is the most level-headed and confident of the students. My instructor pointed me out (omg this is going to be something really cool or something really horrible.) Actually it was both.

KG was my patient and my job was to sit with him. That's all.. provide 1:1 nursing care to a 29 year old man, sounds easy right?

OMG how wrong.

KG has some sort of infection the doctors can't figure out where it is tho. He's had blood cultures, urine culture and sensitivity tests, lumbar puntures, chest xrays, CT scans, CAT scans and a MRI. It's been going on for the past five weeks. He has a horrible headache, pain in him muscles, is running a fever. It's like a case for House only House isn't here to figure it out.

Also over the past five weeks he has become increasingly confused and agitated. He is having psychoses and hallucinations and pariniod delusions. He has constant tremors in both hands. Oh yeah he hasn't slept in five days. Despite the meds he was given: Haldol, ativan, phenergan, loeprazolam (benzo) and benedryl. He should have been dead to the world, that is enough to put a bull into a coma, but he never went to sleep.

Taking care of this guy was a real challenge. My goals for the day were: 1. keep him in bed. 2. Talk to him and keep him oriented to time and place. 3. Keep him from pulling out his IV. 4. Figuring out how to get him to pee with a girl in the room.

He talked a bunch... he talked about the boats floating across the room. He talked about how he and I needed to go hunting, and he was referring to me as a guy. He was constantly getting out of bed and going into the corner, rocking like he was a scared little kid, then the next moment he would be singing and laughing. He talked to people that weren't there. He cussed out his little old mother and his fiance and made them both cry. He attempted to eat the bedcovers.

I kept him safe for five hours. It's the hardest day I've had so far in clinicals. Damn I hope those doctors can figure out what he has. It is really sad to see a man in his prime so debilitated. Oh and I never got him to pee lol.

On the positive side you did get picked out as a top student of your class. You must have a lot of patience to deal with that guy, a lot more than I'd have x.x
Maybe you'll solve this medical mystery and become Super Power Nurse Doctor Awesome-o 3000 and get your own sitcom with added emotional problems that come at a rate much greater than any real people have.
If all else fails we may need House, Cox, and the Hardly Boys to solve this one.

What you need after these days is a nice massage complete with happy ending

I do know now that psychiatric nursing is not for me. And I'll take you up on that massage.

Update!

LOL you guys did a good job derailing this time, which I don't mind a bit. Derail to your heart's content. I'm just glad I don't have to clean Westyle's wall.

My Day, (Which was really yesterday) started in the OB ward at Huge Gigantic Always Getting Kink Lost Hospital. It started out a really boring morning. I kept nodding off sitting at the nursing station. The only patient we had was in false labor and about to be sent home.

Enter J. She comes in, and I know the moring just got interesting. They were trying to get her off a gurney onto an LD bed and she keeps pounding on the bed and screaming at her mother and for her mother, at the same time. She's screaming bilingually which is impressive. The real nurse is setting up paperwork and the chick is screaming for drugs, but she is too close to birth for an epidural. We drop some stadol into her IV, it bearly slows her down. She starts screaming she has to push. We check, she's only dilated to a six, but we expect her to go fast.

She is holding her mamma's hand, well clamping onto her whole arm with both hands and I can tell she is hurting her mom, so I go over to the other side and let her grab on me, cuz I'm young healthy and strong. She really is hysterical, which really surprises me because this is her FOURTH BABY. The really sad thing is that this girl is 21. I'm thinking "Chicklet, are you fucking crazy?????" I keep trying to distract her gently and sometimes it works and sometimes it doesn't.

So her water breaks and we call the doctor because we know she is going to go fast now. The nurse and I are holding her legs back as she begins to push. Now after all this caterwauling about needing to push. It comes time to push and she says she can't push. She's only been in labor about 2 1/2 hours so she isn't too exhausted to push. Well the baby decides things immediately and little momma suddenly can't do anything but push. Suddenly her perinium is buldging out and you can see a little strip of dark hair. With each contration, you can see more of the top of the head (this is called crowning) With a total of four pushes the baby slides out effortlessly and begins to immdiately scream her little head off.

While the doc is delivering the placenta, I go with the pedi nurse to check out the baby. She has a good color, is still screaming, and is absolutely beautiful. The mother is in a state of calm finally, but now the baby is pissed off. After being cleaned up and wrapped in a warm blanket, I take the baby over so the new mom can hold her. Guess what? It turns out that the baby is seriously pissed at the world. Any time it is touched, kissed, or moved it screams bloody murder LOL. I think she gets it from her mother.
